How To Report Suspected Insurance Fraud

The Fraud Reporting Form is to be submitted to the Insurance Fraud Investigation Division by insurers and other agencies.

Consumers are encouraged to submit information in writing to the Office or by contacting the Insurance Fraud Investigation Division for additional instructions.
